Women die from complications during pregnancy, childbirth and the puerperium. Most of these develop pregnancy. Other may be present before pregnancy but worsen The main that cause 80% all maternal deaths are severe bleeding, postpartum infection, high blood pressure (preeclampsia eclampsia), unsafe abortion (WHO, 2019). purpose this study was to determine possibility occurs if pregnant women with grade IV hemorrhoids give birth vaginally. method used in research is descriptive form case study. Conclusion: Hemorrhoids not too dangerous, both for fetus, doctor has entered fourth degree, more advisable do a cesarean section Abstrak: Wanita meninggal akibat komplikasi selama kehamilan, persalinan dan nifas. Sebagian besar ini berkembang kehamilan. Metode digunakan dalam yaitu deskriptif berupa penelaan kasus (case study). Hasil: berdasarkan pengumpulan ditemukan responden menderita dimana pada kebanyakan akan dilakukan operasi mengurangi nyeri saat mengejan. Kesimpulan: Hemorhoid terlalu membahayakan, baik bagi maupun janinnya, namun ketika sudah masuk derajat dokter lebih menyarankan takanan
